
Please read and agree to all of the following instructions:
There will be three (3) $6,000.00 scholarships and three (3) $1,500.00 scholarships given out in each of the three regions.
In order to be considered for a scholarship, each applicant must submit the following:
A completed
Application Form.
A letter of recommendation from a school official with knowledge of your achievements and abilities.
A letter of recommendation from the Fire Chief (or Fire Chief's designee) of the member department submitting the application for consideration.
A statement from the applicant discussing the applicant's goals for his or her life and how he or she feels continued education will help achieve those goals.
A certified or official transcript from the applicant's high school.
Applicant must be a graduating High School Senior this year (2010).

Postmarked no later than March 15, 2011.
The application form should be filled out completely and all questions should be answered in full. If you do not have sufficient room on the application to answer the question completely, you may attach additional sheets of paper.
All references to "you" or the "student" contained in the application refer to the applicant and not to the applicant's parents. If you have a legal guardian (other than your parents), all references to your parent(s) refer to your legal guardian. The requested financial information should be provided by your legal guardian.
The financial information provided on the form will be kept confidential.
If your parents are divorced, you must still provide financial information from both of your parents.
